This document is an example permission slip for parents to sign when children go on outings. All required information is included in the form.

• names of adults and children involved; the time and date of the excursion;
• the location and method of travel; assessment and management of risk; adult:child ratios;
• evidence of parental permission and approval of adult:child ratios for regular excursions;
• evidence of parental permission and approval of adult:child ratios for special excursions; the signature of the Person Responsible giving approval for the excursion to take place.
